Every scout and every adult attending a Pack 160 activity needs a current Annual Health and Medical Record on file. It is good for 12 calendar months from the date signed.

For most pack meetings, den meetings and weekend outings, Parts A and B are all you need. Part C only comes into play for longer camps.

How to submit your forms

Hand them to a leader

Bring the completed, signed forms to any den or pack meeting and give them to your den leader. This is the simplest route and the one we recommend.

Email Pack 160

A note on privacy. These forms contain your child's health information. Pack 160 keeps them only as long as needed, shares them only with the leaders who need them for an activity, and does not post or store them publicly. Please do not email completed medical forms - hand them to a leader instead.

Before an overnight or camp

Plan ahead for Part C

Part C needs a physician appointment, so start early - practices often book weeks out. If your scout has a routine annual checkup coming up, bring the form along and have it signed then.
