Medical forms are due by the October 8th pack meeting

Every camper needs one — each scout and each attending adult. A completed BSA Annual Health & Medical Record, Parts A and B, both sides, with all required signatures. Sports physicals and other physical forms are not accepted, however long you are staying.

Hand them in at the October 8th pack meeting or email them to pack160treasurer@gmail.com.
