Every scout and every adult attending a Pack 160 activity needs a current Annual Health and Medical Record on file. It is good for 12 calendar months from the date signed.
What you need to complete
| Part | Who | Needs a doctor? |
|---|---|---|
| Part A - Informed consent and approval | Everyone, every year | No |
| Part B - General information and health history | Everyone, every year | No |
| Part C - Pre-participation physical | Any event longer than 72 hours, including summer camp | Yes - signed by a licensed provider |
For most pack meetings, den meetings and weekend outings, Parts A and B are all you need. Part C only comes into play for longer camps.
Download the current forms
Always download fresh from Scouting America rather than reusing an old PDF - the form is revised periodically.
How to submit your forms
Hand them to a leader
Bring the completed, signed forms to any den or pack meeting and give them to your den leader. This is the simplest route and the one we recommend.
Questions about medical forms
Not sure what you need, or whether your form is still current? Ask and we will check.
Before an overnight or camp
Plan ahead for Part C
Part C needs a physician appointment, so start early - practices often book weeks out. If your scout has a routine annual checkup coming up, bring the form along and have it signed then.
